srhr-crestPlease follow the instructions below as you complete all of the information to the best of your ability. A complete registration must contain the following:

  • Personal information
  • Registration for at least 15 Wordshops and 3 Worship services
  • Prayer request
  • Your room accommodation fee of $76.00 per person
  • Orientation
  • Signature(s) in the appropriate sections
  • Every section that is appropriate to you is completed

If you are unsure about a section, complete it anyway. Extra information will not delay your registration but incomplete registration forms will. Record all information on the Retreat Registration Form. This form must be submitted to our retreat offices. Please keep a record for yourself. It is very important that you write legibly, so please do so.

We require that you fill out one form for each attendee (e.g. complete one form for each child, or if both spouses are attending one form must be completed for the husband and a separate form for the wife. Additionally, every person that is part of a group must have a completed form.)

  1. Every attendee must choose and attend an orientation. The orientation will explain the retreat, our mandates and answer any questions that you may have.
  2. You cannot receive your room key until you attend the orientation. Please note, we suggest that you coordinate arriving and attending the orientation with your roommate so that you both/all can receive your room keys at the same time. Though we desire to keep your selected roommate(s), if all parties do not arrive at an adequate time, we cannot guarantee that roommate choice. Youth MUST attend the orientation with their parent/guardian.
  3. Please choose your orientation time from this list and plan to be on time. The Thursday orientation is only for those attending the Pastor’s Retreat. There is no orientation after Saturday, at 10:00 A.M. Early orientation is held at LFC, not the hotel.

  1. Please know that at least one adult must be in a room with youth. The processing fee for hotel accommodations is $76.00 per person. No room request/retreat registration is complete until the hotel accommodation form (Section 4) is properly filled out, signed and your $76.00 per person (regardless of age), processing fee is received. It is required as part of a complete registration.

    1. Section 7: Retreat Code of Conduct and Standard
     Please read the Code of Conduct and Standard and print and sign your name in the appropriate section on page…. Doing so states that you are in agreement with our Code of Conduct and Standard and that you understand that any infraction of our rules is ground for immediate removal from the retreat premises and you will not be given a refund.

    Ministry Standard of Conduct

    Due to the nature of the subject matter, the highest level of confidentiality will be adhered to. Therefore, no cameras or taping devices of any kind including camera phones will be permitted in any area including worship services, seven wonders room, and WordShop rooms, for use by any unauthorized personnel. LFCM does not endorse or support any form of relationally or sexually deviant/immoral behavior, i.e., conversations, gestures, homosexuality, lesbianism, bestiality, or incest. We have made a commitment to serve as vessels that God can use to express His unconditional love, hope, healing, and complete restoration to both the victim and the victimizer. Therefore, we ask that all retreat participants be sensitive to the ministry of the Holy Spirit, and that our behavior be based upon holiness and accountability, honoring the name and the character of Jesus Christ, and be governed by the Word of God. Any infraction of these rules is ground for immediate loss of LFCM provided accommodations and removal from the premises. The nature of this retreat requires a no tolerance stance on inappropriate behavior and activity that is not in accordance with the holiness of scripture. LFCM is committed to the healing and restoration of all its guests and wants to ensure an environment that will provide the atmosphere that the miracles, signs and wonders that God intended can manifest.

  1. Please enter the code for the wordshops separated by a comma. Each wordshop and worship service is assigned a code that must be entered below. (See instructional manual for wordshop list and page (8) for complete instructions before completing this section.)

This may include roommates' information, your list of medications, healing square purchase, or corrections from any of the sections. PLEASE!!! - do not ask any questions in this section. The individuals that will process your registration form are not the same individuals that will be answering your questions. Call or e-mail your questions if you have any. Also, please clearly indicate what section your correction/additional information belongs.

  1. Without a prayer request your registration form is incomplete, and this could cost you money. Please write your prayer request in this section. You may attach paper if you need to, but please do not leave this section blank. Your request can be as specific as possible and will be kept confidential.
